National Transition Support Team

The Transition Support Programme has recently launched a new website with lots of useful resources about Transition. The Transition Support Programme ran from October 2008 – 31 March 2011. This website contains all resources produced as part of the programme.

It includes various resources: case studies, lots of NTST resources, guidance, useful links & regional transition resources.

Transition: Bridging the Gap

The purpose of Transition Planning is to help young people achieve their goals. The purpose of this pack is to flag up key issues for young people and their families around Transition and to help families start the planning process. By dipping in and out of the pack, as the young person develops and their needs change, families should have access to all the support and advice they need to allow for a successful Transition.
